Diabetes Clinic
Lead by: Specialist Diabetic Nurse/Dietician/Podiatrist/Dr Leeman
Time: Tuesday 1:30 - 4:30 pm
This Clinic is run by a specialist diabetic nurse in conjunction with Dr Leeman, a dietician and a podiatrist. If you are diabetic and are due an annual review you will receive an appointment. If you have not had an appointment to review your diabetes in the last 15 months please contact us.
Baby Clinic
Lead by: Dr Humphrey
Thursday 1:30 - 2:30 pm
The baby clinics are by Dr Humphrey for 8 week child development checks and immunisations and to allow an opportunity to discuss other issues. Please contact us to book before your baby is 8 weeks old.
Heart Disease
Lead by: Dr Myint & Practice Nurse/HCA
Wednesday 2:00 - 3:30
This Clinic is for annual review of patients with Coronary Heart Disease. Appointments are sent when your review is due. If you believe you should be reviewed in this clinic but have not had an appointment in the last 15 months please contact us.
Epilepsy
Lead by: Dr Islam
Annual review for patients with epilepsy. If you believe you should be reviewed but have not had an appointment in the last 15 months please contact us.
Minor Surgery
Dr Leeman and Dr Myint carry out minor surgical procedures at the Surgery by appointment, all the doctors will be happy to advise you on this.
Ante Natal Care
Patients are seen by the doctors during normal surgery.
Asthma Care
This is run by a practice nurse in conjunction with Dr Islam.
Family Planning
Contraceptive advice is provided by all the doctors or practice nurses during surgery hours.
Smoking Cessation
These sessions are offered either by our Practice Nurses or a trained Smoking Cessation Advisor
Sick Certificates - The Law
Period of sickness of one to three days - you do not need a certificate. For periods of four to six days - your employer may require you to complete a Self-certification Form SC2 which is available from your employer or the DSS. If your employer insists on a sickness certificate for this period there will be a charge which should be paid by your employer.
For illness longer than six days - you will need to see a doctor for them to issue a sickness certificate and you may need to see them for any subsequent renewal of the certificate. The only exceptions to this may be after hospital in-patient or out-patient attendance or for long-term sickness.
