How to Register
The map below shows our practice area. this is an indication only, if you live near the borders please contact us to confirm if you are inside our area.
To register with the Stansted Surgery, you will need to call in at reception and complete a GMS1 form and a new paient registration form (you can download this in pdf format from the document section below) for each member of your family. If you have a medical card please bring that with you. You will also need to provide photographic proof of identification together with confirmation of your residential address. If you do not have photo ID you will need to provide two alternative documents. (Documents you could use include: Birth certificate, Marriage certificate, Medical card, Driving Licence, Passport, Local Authority rent card, Paid utility bill, Bank statements/cards, National Insurance Card.)
You will not be registered with a specific doctor, as we are a group practice, but you may specify which doctor you would prefer to see when you book an appointment and if there is an appointment available you will be offered it.
