For all advice and information regarding foreign travel please make an appointment with the practice nurse 4-6 weeks before departure so that immunisation can be completed. Please complete the below form and hand into reception prior to your appointment.
Please note that a charge will be made for certain immunisations and vaccinations which are not covered by the NHS. A list of these charges is held at reception.

Period of sickness of one to three days – you do not need a certificate. For periods of four to six days – your employer may require you to complete a Self-certification Form SC2 which is available from your employer or the DSS. If your employer insists on a sickness certificate for this period there will be a charge which should be paid by your employer.
For illness longer than six days – you will need to see a doctor for them to issue a sickness certificate and you may need to see them for any subsequent renewal of the certificate. The only exceptions to this may be after hospital in-patient or out-patient attendance or for long-term sickness.
These sessions are offered either by our Practice Nurses or a trained Smoking Cessation Advisor
Contraceptive advice is provided by all the doctors or practice nurses during surgery hours.
This is run by an Advanced Nurse Practitioner and a Practice nurse.
Patients are seen by the doctors during normal surgery.
Dr Leeman carries out minor surgical procedures at the Surgery by appointment, all the doctors will be happy to advise you on this.

This Clinic is run by a specialist diabetic nurse in conjunction with Dr Leeman and a dietician. If you are diabetic and are due an annual review you will receive an appointment. If you have not had an appointment to review your diabetes in the last 15 months please contact us.
