Social Prescribing Service

Social prescribing is a service that helps people connect with services that help with any practical, social or emotional needs that affect their wellbeing with the help of a social prescriber link worker.

It can be used by all ages and is designed to help with complex social needs that affect their wellbeing such as debt – financial stress, poor housing, physical inactivity, grief, care responsibility and unemployment, long term health conditions, mental health issues and people who are lonely or isolated.
